Kentucky Emergency Physician Named 2011 Emergency Department Director of the Year; Finalists Announced from Nebraska, New Jersey and New York

Orlando, Florida, April 24, 2011

Blue Jay Consulting and the Emergency Medicine Foundation (EMF) today announced that Royce D. Coleman, MD, FACEP, has been named the 2011 Emergency Department Director of the Year.

Dr. Coleman, medical director at the University of Louisville Hospital and associate professor at the University of Louisville, department of emergency medicine, will be honored at the Emergency Department Directors Academy at the Omni Dallas Hotel at Park West at 6:30 pm on Monday, May 2, in Dallas, TX. Three additional finalists will be honored later this year.

"The Blue Jay Consulting/Emergency Medicine Foundation Emergency Department Director of the Year Award recognizes current emergency department physician leaders who made significant impacts on improving the operations of their departments, resulting in improvements in the quality of patient care," said Jim Hoelz, chief nurse executive/managing partner at Blue Jay Consulting.

"This year's winner, Dr. Royce D. Coleman was chosen for his active collaboration with nursing to achieve departmental improvements, his commitment to providing excellent patient care, his work to implement programs to improve the quality of the patient experience and community involvement, as well as professional involvement with the American College of Emergency Physicians," said Mr. Hoelz.

An Emergency Department Director wears many hats," said EMF Chair, Michael Gerardi, MD, FACEP. "Daily they are charged with providing leadership; planning; ensuring ethical practices; coordinating with nurses and administrators; improving utilization of the facility, service and staff; all while ensuring quality care for the patients. We are pleased to honor Dr. Coleman, as he truly set himself apart; and demonstrated his dedication to improvement and sustaining excellence in both quality of care and employee and physician satisfaction."

The three finalists in alphabetical order are:

Dave A. Holson, MD, MPH, with Mount Sinai Services at Queens Hospital Center. Dr. Holson's expertise was showcased when Queens, New York, became the epicenter of the outbreak of the H1N1 virus in New York City. As director of the department of emergency medicine at the Queens Hospital Center, Dr. Holson and his team showed outstanding medical and emergency management, and preparedness during this turbulent situation.

Robert L. Muelleman, MD, FACEP, is the emergency medicine chair, department of emergency medicine at the University of Nebraska Medical Center in Omaha, NE, and emergency medicine medical director at The Nebraska Medical Center in Omaha. Dr. Muelleman's leadership and vision enabled him to grow the emergency department at the University of Nebraska Medical Center during the past 12 years from a department under the umbrella of the department of surgery, into an established, full-fledged, resident-based, research-oriented, clinically-focused department of emergency medicine.

Mark Rosenberg, DO, MBA, FACOEP-D, FACEP, is an emergency physician and medical director at St. Joseph's Healthcare System in New Jersey. Dr. Rosenberg formed a unique Emergency Department Executive Committee that meets weekly to discuss and plan everything related to the department. Having collective physician leadership (chair, chiefs of adult and pediatric emergency medicine, residency director) and nursing leadership (director, manager, educators) together enables the department to reduce friction, inform colleagues, share successes, and acknowledge weaknesses. This unified group of nurses and physicians creates cohesive policies supported by the department's leaders that can then be disseminated into all areas of the emergency department.

A panel composed of appointees from EMF and Blue Jay Consulting chose the award winners. The award will be presented by Mr. Hoelz; Mark Feinberg, managing partner at Blue Jay Consulting; and Jay Kaplan, MD, FACEP, chair-elect of EMF and member of the Board of Directors of the American College of Emergency Physicians.

Four Nurses Named Fellows of the Prestigious Academy of Emergency Nursing

Des Plaines Ill. -- The Emergency Nurses Association (ENA) has announced the 2010 inductees to the Academy of Emergency Nursing (AEN). The AEN fellowship is a prestigious honor recognizing emergency nurses for their contributions to patient care in and out of the emergency department (ED).

The Academy of Emergency Nursing was constituted on September 28, 2004, to honor nurses who have made specific, enduring, substantial, and sustained contributions to the field of emergency nursing; who advance the profession of emergency nursing, including the health care system in which emergency nursing is delivered; and who provide visionary leadership to the Emergency Nurses Association (ENA).

Health Central Hospital Decreases Emergency Department Patient Wait Times and Enhances Patient Satisfaction Scores

Blue Jay Consulting Empowers Health Central’s Emergency Department To Optimize in 7 Months

Orlando, Florida, June 1, 2010

Health Central hospital decreased Emergency Department (ED) patient wait times and enhanced patient satisfaction scores as a result of its recent Emergency Department optimization, powered by Orlando-based Blue Jay Consulting.

Blue Jay Consulting provided Health Central's ED staff with a solid foundation of structure and processes for ED operations that improved the patient experience upon arrival to the Emergency Department all the way through their discharge, while decreasing patient wait times across the board.

With new practices in place, patients can be triaged within 8 minutes and be in a room in 25 minutes. The average overall wait time for a patient to see a provider is now 49 minutes -- lower than other major hospitals in the region. For patients experiencing chest pain, the average time to have an EKG has been reduced by 78%.

The industry standard Press Ganey score for Overall Ranking at Health Central hospital reached 78%; nurse ranking went up to 88%; while the provider ranking reached 98%.

Revising the triage process, while implementing immediate bedding and enhancing Health Central’s fast track capabilities has enabled the staff to shorten the length of stay for patients by 17% to 199 minutes. Patients who left without being seen dropped by 74% to only 1.3%. Blue Jay Consulting also incorporated bedside registration and other methodologies to increase and improve patient flow.

The award winner and finalists were chosen from nearly 80 nominations from across the country, including some overseas, by a selection panel composed of appointees from Blue Jay Consulting and the Emergency Medicine Foundation.

The winner and finalists demonstrated significant contributions to their emergency department in the following categories: quality patient care, operational effectiveness, education, and community service. They demonstrated collaborative relationships with nursing and ancillary departments to implement and improve operational and clinical standards based on evidence-based practice. They also stood out for their abilities to create and sustain high degrees of patient satisfaction, while implementing creative and innovative strategies to address emergency department throughput.

Linda Lawrence, MD, FACEP, EMF board member, will honor the winner on April 26, 2010, at the American College of Emergency Physicians Emergency Department Director's Academy, Phase II.

This year's winner, Rex G. Mathew, MD, FACEP, is vice president of emergency medicine clinical operations at Thomas Jefferson University Hospital, a Level 1 Trauma Center in Philadelphia. Dr. Mathew displayed his leadership abilities, combined with his clinical knowledge, to work with leaders throughout the hospital to improve care in the emergency department. He fulfills a unique roll for the department and the hospital as both an administrator and a practicing physician. Having his "feet in both doors" has enabled him to make a positive impact on his patients and fellow clinicians. In addition, Dr. Mathew was instrumental in developing many quality improvement projects including the newly accredited Thomas Jefferson Chest Pain Center.

"The emergency department director not only sets the tone and direction for the emergency department, but serves as the point person to satisfy patient's needs and provide safe, quality care,"said EMF Chair, Alexander Rosenau, DO, FACEP. The two organizations applauded this year's finalists who are listed alphabetically:

Patrick J. Crocker, DO, MS, FACEP, is the chief, of emergency medicine at Dell Children's Medical Center of Central Texas in Austin, TX. Dr. Crocker was instrumental in working across disciplines to create the Comfort Zone Program, which addresses the comfort, anxiety and pain perception of patients. In addition, he worked closely with emergency nursing leadership to adopt high standards of professional performance and conduct.

William Dalsey, MD, MBA, FACEP, is the chairman of the department of emergency medicine at Kimball Medical Center in Lakewood, NJ. Dr. Dalsey's collaborative approach to patient care earned Kimball Medical Center top honors for the last five years in patient satisfaction scores by Press Ganey. In addition, Dr. Dalsey and the nurse manager transformed emergency operational efficiency to reduce door-to-doctor times, as well as overall throughput while never compromising quality. His "no wait ED" was one of the first of its kind.

Paul Ernest Pepe, MD, MPH, FACEP, is the chief of emergency services at Parkland Health & Hospital System in Dallas, Texas. Dr. Pepe's strong team philosophy stood out in his application. Both he and Jennifer Sharpe, RN, director of nursing, were recognized because they epitomize the work ethic and philosophical temperament that guaranteed the tremendously successful journey to better patient care and enhanced community service at one of the most visible emergency care centers. Dr. Pepe is an award-winning physician who serves as an inspirational mentor.

Blue Jay Consulting and the Emergency Medicine Foundation announced today that they are now accepting nominations for the newly created Outstanding Emergency Department Director of The Year Award. Current emergency department physician leaders who have made a significant impact on improving the operations of his/her department, resulting in improvements in the quality of patient care can be nominated. The announcement came today, preceding the start of the American College of Emergency Physicians Scientific Assembly in Boston.

Any member of the American College of Emergency Physicians (ACEP) or hospital leader may nominate an ACEP member for this award by completing the nomination form. Forms are available to download online at www.bluejayconsulting.com or www.emfoundation.org.

Other criteria for the award include that the nominee is an Active or Life member of ACEP and must currently be in a leadership position in an emergency department. The nominee must demonstrate significant contributions to the department in the following categories: Quality Patient Care, Operational Effectiveness, Education, and Community Service. The nominee will demonstrate collaborative relationships with nursing and ancillary departments to implement and improve operational and clinical standards based on evidence-based practice. The nominee will create and sustain a high degree of patient satisfaction with emergency care delivery and will implement creative and innovative strategies to address emergency department throughput.

All entries must be completed and received via e-mail or mail no later than Monday, February 1, 2010 for consideration by the selection panel.

The selection panel, which will consist of appointees from the Emergency Medicine Foundation and Blue Jay Consulting, will judge the entries, with its recommendations subject to review by the EMF Board of Trustees. The decision of the Board is final, and the Board reserves the right to make no award if there are no nominees of sufficient merit. The awardee must be present at the Emergency Department Directors Academy, Phase II, on Monday, April 26, 2010 in Dallas, TX to receive the award. If the awardee is not scheduled to attend this course, travel arrangements will be provided.

Assembling Leadership Skills and Trauma Care are Two Keys to Pediatric Nursing

Jeanne Venella, RN, MS, CEN, CEPEN and Consultant at Blue Jay Consulting will be sharing her pediatric expertise at the 25th Annual Pediatric Nursing Conference in Orlando, Florida on June 27.

Kicking off the General Session at 8:30 am, Venella will be discussing Assembling Your Leadership Toolbox. It is generally agreed that leadership is essential to the survival of the nursing profession. Venella jokes that leadership is the most visible and least understood phenomenon in the workplace. Diving further into the topic Venella suggests looking at ways to uncover the leader in every nurse we hire and retain, and gives advice on how to deal with the increasingly cynical workforce, the constant changing of job responsibilities, and the increased demands made by organizations. After attending this session, nurses will recognize what tools are needed to coach, teach, and mentor future nursing leaders.

At 1:15 p.m. Jeanne Venella enlightens the audience on a topic called Why Does It Always Happen on the Way to School? This session will discuss and review typical and atypical school accidents and injuries that occur on the way to school. Using true case examples and humor from more than 30 years' experience in a pediatric emergency department, Venella will review injuries from head to toe, including some that are unusual presentations. Emphasis will be placed on the role of nursing to reduce and prevent these types of injuries.

Formerly on the National Pediatric Disaster and Preparedness Committee at the American Academy of Pediatrics, Venella is well known for her expertise in the specific and unique needs of children in disaster situations. Venella has spoken before at the Pediatric Nursing Conference which is designed to focus on the specific needs of nurses at the point of care in order to enhance nursing knowledge, skills, and evidence integral to pediatric nursing practice.

A lot is being asked of emergency departments

Regarding the article "Emergency department crowding a lingering problem, GAO report says":
Working in emergency departments for more than 30 years, I have been witness to the struggles that the GAO report mentions. As a senior consultant working with emergency departments across the U.S., I know they are working hard to be the safety net for the medical community. Unfortunately, emergency departments are challenged with less and less resources to handle the increasing volume and acuity of patients who are older, sicker and often without primary care. On top of that, there are fewer physicians and nurses being produced due to the cuts in federal funding, which further increases the burden placed on staffing emergency departments. While emergency departments across this country are trying diligently to improve each process they do, standards for inpatient care because of boarding are being mandated.

Examples include: chest pain timelines, i.e. ECG, in 10 minutes; stroke centers, i.e. CT, in five minutes; and meds in 30 minutes.

However, there is good news. Many departments are working hard to put proper efficiencies in place to allow more patients to be seen -- even with fewer resources. Hospital administrations that see the ED as the front door of the hospital, and place a significant challenge to all leaders of the institution to see the ED flow and intake as a strategic measure have the greatest successes.

Unfortunately, the days of getting in and out of emergency departments in an hour are in the past. We are challenged to give primary care to millions of patients who have no primary-care specialists and need medical care.

Wait Time Lowered To Less Than 30 Minutes at Mon General Hospital's New Emergency Department

Blue Jay Consulting Empowers the Staff to Optimize Emergency Department

Patients of the Mon General Hospital in Morgantown, West Virginia can now see a physician in the emergency department within 30 minutes. The time to triage has been lowered from 22 minutes to 8 minutes; time to bed decreased from 26 minutes to 13 minutes and time to see a physician fell from 45 minutes to 30 minutes. The optimization paid off in patient satisfaction as well.

"From the time of registration to the point of discharge, my wife and I were pleased to observe the administering of hands-down, first rate treatment," said recent patient David Barton. "Facilities were clean, operations were orderly and all personnel we came in contact with were most competent and showed signs of caring," Barton said in a recent letter sent to hospital administrators.

These enhanced measurements are due to the performance optimization conducted by the ED experts at Blue Jay Consulting in conjunction with the hospital's Emergency Department staff.
As more people in the area seek quality emergency care, Mon General Hospital's ED was getting busier. The department was meeting industry standards, including high Press Ganey scores, and had low walk out rates. The organization was operating smoothly but the executives wanted to distinguish themselves in the community, while becoming more competitive.
After an in depth analysis and specific recommendations, the hospital executives selected Blue Jay Consulting to implement a strategy that would match the new state of the art facility while delivering unmatched care.

Nancy Bonalumi, RN, MS, CEN, Blue Jay Consulting Senior Consultant, was onsite leading the project. She implemented the Emergency Severity Index (ESI) 5 level triage process, a reliable and valid tool for sorting patients upon arrival. Prior to 'going live,' all registered nurses attended a course to learn and understand this process. Within a week of implementation, key operating statistics in the ED made dramatic improvements due to the staff's commitment and understanding of ESI.

The second major change implemented was the immediate bedding process. Patients are placed in an available bed, rather than being interviewed at the front desk. With this no delay system, patients can be given care and information can be taken simultaneously, not consecutively. As the day progresses and gets busier, the staff evaluates the flow of patients and works to try to stay a bed ahead. The goal is to move patients in quickly, get them the quality care they need, and move them into other parts of the hospital if necessary or discharge them.

Blue Jay Consulting also demonstrated ways for the ED staff to further analyze data which would optimize the department's staffing needs.

The final major optimization technique was to develop committee structures. Steering committees and work groups, supported by Blue Jay Consulting, enabled staff (physicians, nurses and ancillary staff) to make choices and recommendations to the department leadership. This empowered staff to make decisions about their work. They own it, but Blue Jay consultant's stay on site for six months to assist the staff in making the changes become part of the department's normal operating procedures.

Again, the pay off was high. The Centers for Disease Control and Prevention reported recently that the average ER wait time is one hour, but with these improvements, Mon General Hospital is well ahead of this average.

Mr. Stewart holds a Bachelor of Science in Nursing from Texas Tech University Health Science Center and is currently in the Master of Nursing Administration Program. Executives at Dartmouth-Hitchcock Medical Center are always working to improve their level of service. They brought in Blue Jay Consulting to work with DHMC's Emergency Department to enhance interim leadership and optimize performance. Blue Jay Consulting helped DHMC to improve operational processes, redefine roles and responsibilities, and improve communications methodologies.

A comprehensive assessment identified opportunities for improving patient throughput in the Emergency Department, beginning with data management and tracking, enhancement of the charge nurse role and immediate bedding when Emergency Department beds were available.
A detailed dashboard focusing on key operating metrics including length of stay, acuity, patient satisfaction, LWBS and AMA rates was developed and reviewed at weekly interdisciplinary leadership meetings, as well as at monthly physician and staff meetings. Immediate bedding when beds were available was implemented. Modifications to prompts in the Emergency Department tracking system improved the ease for physician staff to identify patients ready to be seen. Concurrent with strategies for improving patient flow, routine charge nurse meetings were held to enhance communication and accountability of that role. A 360° performance appraisal tool for charge nurses was implemented to provide specific feedback to individual staff.
The results are improved patient care. Time to bed decreased 35%, from 55 minutes to 36 minutes. Time to MD was reduced 38% from 99 minutes to 62 minutes. The length of stay for discharged patients also improved 22%. Patient satisfaction regarding waiting time to treatment room rose by a mean score of 15.8 points, from 72.9 to 88.7.

At the recent Pennsylvania ENA State Meeting, Blue Jay Consultant, Jeanne Venella, RN, MS, CEN, lectured on Pediatric Triage and Approaches with Children. At the 24th Annual Pediatric Nursing Conference, June 25-29th, in Las Vegas, Ms. Venella will speak about Nickel Mines, the Story of the Amish School Shooting, as well as Assembling your Leadership Toolbox.

JoAnn Lazarus, RN, MSN, CEN, VP Operations-Emergency Services at Blue Jay Consulting spoke recently at the Kansas City ACHE meeting about Current Trends in Emergency Care. She also served as a panelist at this meeting.

Blue Jay Consulting Enabled the Emergency Department At Capital Regional Medical Center to Deliver Outstanding Service to Patients in 10 Minutes or Less

Orlando, Fla., June 10, 2008 -- World class Emergency Department consulting firm, Blue Jay Consulting, LLC enabled Capital Regional Medical Center in Tallahassee, Florida to decrease Emergency Department patient wait times to ten minutes or less. In addition the team of experts improved patient throughput, decreased the amount of "left without being seen" patients, and enhanced patient satisfaction scores.

In pursuit of this 198-bed hospital's commitment to improve the quality of healthcare in its community, the management team recognized the need to enhance its Emergency Department and charted Blue Jay Consulting with that task.

Seeing over 50,000 patients a year, the hospital had high lengths of stay, hold times and walk out rates. The goal was to focus on throughput by getting patients to an ED bed, discharging patients or moving them upstairs to a room in a more timely fashion. Communications, work flow, and education issues stood in the way.

Meetings with staff and the ED Nursing and Medical Directors, along with observations of processes, provided Blue Jay Consulting with the information to move forward. Based on these findings, Blue Jay determined the strategy that needed to be implemented.

A steering committee was formed made up of staff from the ED to determine what needed to be changed. This provided the hospital with ownership of the project from the beginning.

Blue Jay Consulting, working with the steering committee, implemented processes to improve communications, streamline triage and realign staffing to more effectively move patients through -- avoiding congestion in the ED waiting area. Additionally, to improve flow, the role of facilitator was created to move patients more quickly through the department. The team succeeded and was able to lower the walk out rate from 3.6% to 0.7%. With the national standard less than two percent, this is a tremendous improvement.

Financially, the organization will realize an increase in annualized revenue as a result of capturing these visits. This increase does not include the additional contribution margin from laboratory testing, radiology exams or from an increase in admissions.

Blue Jay Consulting, LLC names JoAnn Lazarus to lead Emergency Services Group

Orlando, Florida, February 26, 2008 -- Blue Jay Consulting, LLC, a leading provider of consulting services to hospital emergency departments nationwide, today announced the appointment of JoAnn Lazarus, RN, MSN, CEN as vice president of operations -- emergency services.

JoAnn will be responsible for the management and development of the Blue Jay consultants working in the emergency care market. She will report to Jim Hoelz, co-founder and managing partner of Blue Jay Consulting, LLC.

JoAnn has been with Blue Jay since its inception in May of 2006. "She has been instrumental in the growth of our company and she is well respected by her peers," said Hoelz. "Her leadership and strategic focus have been keys to our success. We thank her for her past contributions and look forward to her focus on growing our business."

JoAnn is a 35-year veteran of the health-care industry with a broad range of clinical and consulting experience. Prior to Blue Jay, she was with Cardinal Health, where she served as a clinical services consultant. Previously, she was director of client services at EN-CHART, and a clinical analyst and consultant at EmSTAT Corporation.

JoAnn is a long-time state and national leader in the Emergency Nurses Association, where she served as the 2007 secretary/treasurer. In addition, she is an active member of the Texas Emergency Nurses Association. Her nursing degrees include an associate degree, a bachelor of science and a master of science with an administrative specialty in critical care.
